Instructions for filling out Examination Application Forms

  1. Read everything before filling out application form

  2. Please read the Notes on page 2 carefully

  3. ONLY PRINT IN BLOCK LETTERS

  4. Download Course Codes list from below and ensure that the correct course name and code is filled in your form

  5. Please attached current passport size picture on the application form

  6. Submit Application form to the office in Toronto through the method approved and communicated to you by the due date set out

  7. Please submit all examination fees along with your application form

  8. NO APPLICATIONS WILL BE ACCEPTED BEYOND THE DUE DATE SET BY OFFICE
